I realize there may be physical activity involved, and the Venue is at higher elevation, and if I am pregnant, have any medical conditions or mental or physical disabilities I have consulted my physician prior to any included physical activites

If I am I pregnant, have medical conditions or any mental or physical disabilities that may limit participating in certain types of activities I have disclosed these conditions above. I understand Windward Tailor, LLC will do their best to modify activities to still be able to participate, but may not be able to do so due to the requirements of the Venue or Vendor providing the activities

If I have disclosed any medical conditions or mental/physical disabilities in which participating in physical activites may be limited I give permission to Windward Tailor, LLC to share this information with Third Party Vendors in order to access if modifications may be made safely for me to particiapate. I understand if my specific limitations are not able to be safely modified there is no refund

I have alerted Windward Tailor to any and all food allergies and understand notification during registration is needed to accomodate any necessary menu modifications
